본 논문은 임상의사가 환자치료 시 발생하는 문제를 해결하기 위해 온라인상에서 정보검색하는 과정을 연구하였다. 이를 위해 국내 대학병원 내과 전문의 10명을 대상으로 관찰조사와 심층면담을 수행하였다. Wilson의 문제 해결 모형을 기반으로 임상의사가 임상질문 해결을 위해 온라인상에서 검색하는 단계별 특성과 정보검색의 효용성에 대해 분석하였다. 그 결과, 임상의사는 사전 배경지식을 바탕으로 즉각적인 임상질문을 구성하지만, 실제 검색에서는 대부분 기본 검색전략만 사용하였으며, 본인에게 익숙한 특정 정보원만을 선호하였다. 하지만, 온라인 정보검색 평균 만족도는 7점 리커트 스케일 기준 5.7로 높게 나타났으며, 정보검색 후에 문제해결지수가 상승하였다. 임상의사는 잘 정리된 근거 중심의 신뢰성 높은 정보를 빠르고 쉽게 이용하기를 원하기 때문에 환자 진료시스템에서 바로 치료에 필요한 정보를 보여줄 수 있는 검색시스템의 연계가 필요함을 알 수 있다. 또한, 구글이나 PubMed 외에 다른 온라인 정보원에 대한 인지도는 현격히 낮게 나타나 다른 정보원에 대한 적극적인 홍보와 정보활용교육의 활성화도 필요함을 알 수 있다.

Abstract

This study aims to analyze physicians’ online information search process to solve the clinical questions at the point of care. To achieve this purpose, ten university hospital-based physicians participated in-depth interviews and observation studies. Based on Wilson’s problem solving process, this study analyzed the characteristics of each information search stage and efficiency of online searching. The results showed that participants tend to relatively immediately formulate their clinical questions. However, basic searching strategies were only used and a few preferred information sources were chosen. However, average satisfaction degree of online searching appeared high with 5.7 (7 Likert-scale) and problem-solving index increased after searching. As physicians are likely to use well organized and evidenced-based credible information easily, it implies the needs for an integrated search system within the electronic medical record (EMR). In addition, as other online resources’ awareness is lower comparing Google and PubMed, active promotions and training of other resources are needed.

Abstract

Physicians have information needs related to academic research and clinical practice but encounter difficulties seeking appropriate medical resources and information. The goal of this study is to develop a search system to support Korean physicians’ information needs. To access sources to meet the identified need, in-depth interviews were conducted, and MediSearching, a new search system, was developed accordingly. To verify its usefulness and to obtain users’ suggestions, interviews were conducted and feedback was solicited via collected test searches. The initial set of interviews indicated that physicians’ information needs and search behaviors differed by type of hospital and clinical department. Physicians in university hospitals with a greater need for research materials searched for academic articles, whereas physicians in specialty hospitals or primary care clinics with a stronger need for materials related to clinical practice asked their colleagues for information. Consequently, MediSearching was designed to satisfy both groups’ needs, and the test search provided useful search results compared with existing services. Korean physicians have previously had to repeat their search process on separate sites that provide different services according to type of information and search method. MediSearching may reduce this inconvenience and add documents in various formats and languages.
